Conveniently located throughout Bristol County, the Diocesan Health Facilities’ five skilled nursing centers offer modern medicine’s best practices with an emphasis on spiritual well-being. This unique holistic approach, found nowhere else in Southeastern Massachusetts, has made us a leader in our field.
We offer short- and long-term nursing care for adults of all ages – from those recovering from surgery or illness with a goal of returning home to the frail elderly, many who make us their home. Regardless of each individual’s medical profile, the goal is always the same: to restore and maintain the highest level of physical function and personal independence.
We do this by creating customized treatment plans to address each resident’s individual needs. Our skilled nursing team is complemented by rehabilitation therapists, dieticians and social workers to deliver the optimum level of care, no matter how complex the plan. Innovative pain management, palliative care, and access to hospice programs ensure the best quality of life possible, even in its final moments.
Ongoing pastoral care is integral to our approach, with access to on-site daily Mass and a broad range of engaging activities to stimulate personal reflection and social interaction. We are proud to meet the needs of the local community by offering bilingual staff members, religious services spoken in Portuguese at some locations, and activities reflecting the culture.

The Diocesan Health Facilities’ pain management program was instituted to deliver optimum relief and elimination of acute or chronic pain for all residents, regardless of age, illness, or mental health status. A comprehensive pain assessment begins upon the resident’s admission and continues throughout the duration of their stay. Medically advanced assessment tools allow us to provide comfort and care to cognitively impaired residents.
Our palliative care program, offered at each of the Diocesan Health Facilities skilled nursing centers, provides terminally ill residents with specialized treatment, including appropriate and aggressive pain management, and compassionate support. End-of-life care is coordinated by a palliative care team, which may comprise the resident’s physician, nurse, and pastoral care. Special Care Rooms at each of our facilities allow residents and family members to experience a private room with accommodations and conveniences that provide medical services in a serene setting.
